Hospice & palliative care · Lubbock, TX
Hospice nurses, aides, chaplains and social workers who drive to farmhouses in Crosby County as readily as to apartments on 50th Street.
Why we do it this way
When we started in 1991, families west of Lubbock were told hospice couldn't reach them. So our first nurse put a cooler and a map in her truck and went anyway. That is still the job.
Today our teams are based in Lubbock and Levelland. Every patient has a nurse case manager who knows the road to their house, and a nurse on call who will come at night when a phone call isn't enough.
— Dr. Nathaniel Okafor, Medical Director

Sample family reviews
We live forty minutes past Ropesville. The nurse came out at midnight in a sandstorm because Daddy couldn't catch his breath. I will never forget her headlights coming up the drive.
Dr. Okafor sat at our kitchen table for an hour and explained everything twice. Nobody at the hospital had time to do that.
Our chaplain grew up on a cotton farm outside Idalou. He talked weather and crop prices with Grandpa for half an hour before he ever mentioned prayer, and Grandpa asked him back.

Good to know
When a doctor expects the illness to run its course in six months or less, or when treatment has stopped helping. Many South Plains families tell us they wish they had called months sooner. A call commits you to nothing.
Yes. The Medicare hospice benefit covers the care team, medications, supplies and equipment related to the terminal illness, usually at no cost to the family. Texas Medicaid, VA benefits and most private plans cover hospice too.
Anywhere in Lubbock, Hockley, Lynn, Crosby and Hale counties. Call us with the address and we will tell you which nurse covers that road.
Yes. Her doctor can stay her attending physician and work with our medical director, or our physicians can take over her hospice care. It is her choice.
It happens. Patients can leave hospice at any time and return later if they need to. Medicare allows it, and we will help with the transition.
Contact
Our nurse line is answered by a hospice nurse, never a recording, every hour of every day.
