For families

For families

You're part of the plan. Here's how it works.

  1. 01

    Your case manager

One nurse who knows your parent and calls back the same day.

  2. 02

    Family meetings

A sit-down whenever the plan changes, in person or by phone.

  3. 03

    Visit texts

A text the night before each visit, to you and your parent.

  4. 04

    After hours

A registered nurse answers every night and weekend.

Good to know

Questions, answered

Who pays for home health?

Medicare covers home health in full for patients who are homebound and need part-time skilled nursing or therapy under a doctor's plan of care. Most Medicare Advantage plans and Texas Medicaid also cover it.

Which office will care for me?

The office closest to your home. All three share the same nurse line, standards and programs, so you'll get the same care wherever you live.

What does homebound mean?

Leaving home takes considerable and taxing effort, or needs help, a device or a ride. Church, the doctor and the barber are still allowed.

How soon can care begin?

Most first visits happen within 24 hours of the referral, seven days a week.

What areas do you serve?

Gregg, Harrison, Rusk, Upshur and Panola counties, including Longview, Kilgore, Gladewater, Marshall, Hallsville, Henderson, Gilmer and Carthage.

Coming home soon? We'll leave the light on.

Call any office, or ask your doctor or hospital case manager to send the referral to Caddo Pines Home Health.
